Employment / Training Plan

I understand that participation in the SNAP Employment and Training Program is voluntary and will not affect my benefits.

I understand that I must:

• Complete and submit my weekly training progress form.
• Communicate with my Training Case Manager about any changes to my employment status.
• Tell my case manager if I plan to move to another state and would like to continue training to earn my certification.
• Make satisfactory progress toward earning my professional certification.
• Respond in a timely manner to all communications from DCFS and AimHigh Education Staff.

I understand that my not making satisfactory progress and maintaining active participation may result in loss of employment and training services.

If circumstances change and I am unable to carry out the expectations in this Employment Training plan, I will notify my case manager.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD 3027 form or letter must be submitted to:
